Difference Between IBD and IBS

IBD stands for Inflammatory Bowel Disease while IBS stands for Irritable Bowel Syndrome. There are no clear origins and treatments for these diseases. So, they are confused. Both of them cause life threatening complications. Sometimes, they may require surgical treatment. Both of them are equally harmful physically as well as psychologically. Both of them affect the gastro intestinal tract. Both the diseases can be compared in relation to symptoms, patholphysiology, symptoms, etiology, management etc.


IBD is an autoimmune disease having excessive cytokine activity. It has two sub diagnoses- ulcerative colitis and Crohn’s disease. These two types differ in relation with mucosal affliction the pattern of distribution on the mucosa. They are characterized by abdominal pain, diarrhea, rectal bleeding, weight loss, extra intestinal manifestation and vomiting etc. Risks of nutritional deficiency and risk of malignancy are associated with these two. Generally, steroids are given to patient to manage flare ups and immune suppression. Sometimes, surgery may be required. The patient needs regular medication. Complications are always possible.


IBS is associated with infection as a result of stressful life. It is diagnosis of exclusion. There is no specific mechanism with multitude of risk factors. The IBS is of psychogenic origin which is supplemented by the neurogenic sensitivity. The symptoms may range from mild to severe. They include constipation, abdominal cramps, diarrhea, urge to defecate etc. The symptoms occur after a meal. They reduce the bowel movement.

The difference between IBD and IBS

Both IBD and IBS cause the loss of appetite and change in the bowel motility. Specific investigations are needed to exclude the threatening pathologies. The symptoms of both of them include distension, diarrhea, abdominal pain and mucus. The symptoms are associated with anxiety, depression and fibromyalgia. They are worsened during the menses. IBD is autoimmune condition while IBS is aetiology that is still surrounded by mystery and is more associated with psychology. No visible pathologies are there in IBS while IBD is characterized by many pathological changes. IBD does not have alternating diarrhea and constipation while IBS does. IBD causes rectal bleeds, strictures and fistula etc. IBS does not show extra intestinal manifestations while IBD does.
